• 12 In-Home Visits Annually — schedule one house call per month; you may accrue visits within the calendar year, but they do not roll over.

• 36 Virtual Consultations — book up to three quick 15-minute sessions each month for follow-ups or brief concerns; unused visits expire at year’s end.

• When you reach your 12 in-home visits or 36 virtual consultations, your membership counts as fulfilled, and the full year’s membership fee becomes due immediately.

• You may accrue visits and consultations during the calendar year; however, none roll over into the next year.

• Each virtual consultation lasts 15 minutes, ensuring efficient support.

• After-hours coverage requires an additional fee and is not included.

• Controlled substance prescribing begins at the Premier tier and continues with higher plans.
