What’s Included in the Select Plan Monthly Membership

With 6 in-home visits per year, you receive hands-on care every other month. In addition, you stay connected between visits through virtual consultations and priority scheduling. Consequently, you never face the frustrations of long waits or rushed appointments.

6 In-Home Visits Annually — schedule visits every other month; accrue them within the calendar year, but use them before year’s end since they do not roll over.

24 Virtual Consultations — book 15-minute sessions throughout the year for quick guidance or follow-ups; however, unused consultations expire at year’s end.

Comprehensive Annual Physical — complete a detailed physician exam and walk away with a personalized plan of care.

Medication Management — receive safe prescribing and oversight for ongoing therapies (excluding controlled substances).

Mobile Lab Testing & Imaging — request bloodwork, X-rays, ultrasounds, and more at your location, ensuring you avoid unnecessary clinic visits.

High-Priority Scheduling — secure appointments quickly, so you gain faster access to care when you need it most.

⚠️ Only Diamond and Executive Members enjoy direct physician access by phone, text, or video. As a Select Member, our office staff coordinates all calls and scheduling for you.

Important Select Plan Monthly Membership Policies

To protect fairness and clarity, we outline the following policies:

• Once you reach your 6 in-home visits or 24 virtual consultations, we charge the full year’s membership fee immediately.

• You may accrue visits and consultations within the year; however, they do not roll over.

• Each virtual consultation lasts 15 minutes and covers follow-ups or brief concerns.

• After-hours coverage requires an additional fee, so you stay in control of your healthcare costs.

• Controlled substance prescribing is not available at the Select tier; however, this option begins with the Premier Plan.
